Student Accessibility Services

To initiate a Request for Accommodations:

• Please complete the Student Accommodations Request Form.
• Disability Documentation Should Include:
  • Diagnosis including date of diagnosis
  • Signature of the clinical or medical provider with credentials
  • Recommendations for accommodation & functional limitations
• Students are more than welcome to use the “Physician’s Form”
• Read the Rights and Responsibilities document
• Log into the Accommodate Portal to complete the Accommodation Process
  1. Log in to Accommodate
     • Select Student
  2. You will arrive in the Home section
  3. To view your initial accessibility request (made through the public-facing portal), select Accommodation > Accessibility Request​​​​​​​
     • Make sure you import your disability documentation to the system at this time. For additional support on the process, please see the instruction in the forms and documents section below entitled: “How Students Upload Documents in Accommodate”
  4. If this is your first time seeking accommodations here at UMFK, or if you are a returning student but would like to meet to discuss your accommodations, please make sure to schedule an appointment with the Accessibility Manager.
     • To request an appointment, select Appointment Request > Request New Appointment
